To provide practical information, skills, and support to enable informed decision-making and help manage diabetes.

Educational Session Options:
|
|
|
|
|
Group Sessions The group education series includes up to 5 sessions. Each 2-hour small group session is designed for individualized learning.
|
|
|
Each group discussion topic includes:
- 1 hour - discussion on the listed topic
- 30 minutes - hands-on demonstration and work
- 30 minutes - participant goal setting
|
|
|

Care for Life A refresher/update session is offered for those who have had some education before. Topics covered are based on patient need assessment and goals.

Individual Sessions are available for patients with special needs such as hearing, visual, or cognitive deficits, language barriers, or insulin instruction needs.
